Join SAC-CCHL and Emerging Health Leaders in exploring tools and strategies to support complex care planning for Albertans. 

What can be done about predicting patient needs in the future?  How can we better balance the needs of the population for preventing and treating mental health, diabetes and cardiovascular disease? How do we evaluate our strategic choices and impact on patient care?  What tools are available to test our decisions before investing? These questions and more will be explored by this KPMG sponsored event, which will provide live demonstrations of the international tools, models and methodologies used to inform strategic decision making for chronic disease.  A live demonstration of the UK’s National Health Service Long Term Conditions Commissioning and the Year of Care Funding Model, the vehicle driving transformation change, and how this program engages patients and primary care providers, in addressing individual needs at the local level.
